St. Paul Elder Services has developed specialized, community-based programs and facilities that are specifically designed to enhance the lives of aging individuals. Whether the need is for post-acute care or long-term support, our caring, trained personnel are here to help either on our Kaukauna campus or in your home in the community. From support services we can bring to your home to rehabilitation centers for recovery, we can meet your needs and expectations.

The following programs and services are operational during this COVID-19 pandemic:
St. Paul Home — Long-term care and skilled nursing services, specialized in memory care.
Center of Rehabilitation — Inpatient physical, occupational, and speech therapy and skilled nursing services, and outpatient therapy services.
St. Paul Villa — Assisted living facility.
St. Paul Manor — Memory Care assisted living facility.
St. Paul Hospice — Palliative and hospice care, provided both in-home and on our Kaukauna campus.
St. Paul At Home — In-home supportive and companion care, meals on wheels, transportation services, emergency response systems, and other tele-health services.
Passages Memory Care Program — Dementia support services at home, within our Kaukauna campus, and through virtual consultations.
